PDF Form Info
Some of our forms are provided as downloaded PDF files for you to complete and then return to us. This page provides general information and resources to assist you with these forms.
Most web browsers will display our forms directly and allow you to download them to your computer. But only some web browsers allow you to fill in the form on your computer and in those cases you must “save as PDF” to preserve the inputs. Then you can email the completed form to us at firstname.lastname@example.org.
PC or Mac
You can also save the PDF form on your PC or Mac and complete it with another program. Some PDF reader programs will save your inputs along with the downloaded file while others will not. In cases where the program does not save your inputs, you may also be able to “save as a PDF” to preserve them and email the completed form to us at email@example.com.
But in some cases you cannot save your inputs with the file or may not have the option to “save as a PDF.” In these cases you should complete the form either by:
- fill out the form on your PC (if possible) and print the completed form; or,
- print the blank form and complete the required information by hand.

Some free PDF reader programs
Adobe Reader – some versions may not save your input so when you save the file so you may need to print the completed form and mail to us or scan the form and then email it back to us.
Foxit Reader (then click “Free Software” and then “Free Download”) – will allow you to save your inputs, so you have the option of sending us your forms via email.
